Perihal : Usulan Paska PSBB Langkah Kedua Total Case Finding.
Langkah Ketiga Membuat Obat dan Vaksin.

Salam sejahtera Bapak Menkes, keluarga dan Jajaran ytc,

Izinkan saya menyampaikan surat kedua.
Sebelumnya juga melalui WA (maklum Work From Home), telah saya tuliskan surat dengan perihal PERCEPATAN PENERBITAN PENETAPAN PSBB tanggal 7 April 2020, sebagai LANGKAH Pertama.

Saat yang sama bapak menerbitkan persetujuan atas usul Pemprov DKI Jakarta berstatus Pembatasan Sosial Berskala Besar.

Sebelum masuk ke substansi surat kedua. Izinkan kita menatap implementasi PSBB wilayah Jakarta. Sudah lebih seminggu penerapan PSBB sejak 10 April yang lalu. Sepertinya Jakarta “dibiarkan” jalan sendiri, padahal Pemerintah yang buat Pengaturan. Dalam kegawatdaruratan Pemerintah Pusat dan Pemerintah Daerah bekerjasama.
Dalam sistem Otonomi Daerah, fungsi Pusat tidak boleh berkurang sebagai konsultan, pendampingan, pengawasan, penguatan dan bahkan pembiayaan.

Alhamdulillah dalam 2 minggu Bapak Menkes sudah menerbitkan 17 Keputusan persetujuan penetapan usul Daerah dengan status PSBB pada 10 Kota dan 6 Kabupaten, diawali Provinsi DKI Jakarta.

Para ahli dari surveilans epidemilogi melihat ada 50 dari 100 Kabupaten/Kota memiliki resiko tertinggi dengan 49%, beberapa diantaranya berada di Pulau Jawa yang dapat dipertimbangkan dalam status PSBB.

Dalam surat pertama, kami menyarankan metode penetapannya lebih aktif dan cepat dengan memanfaatkan kemampuan tehnologi IT dengan melakukan pemetaan kepadatan dan resiko kasus (Red zone).

Aktif dan cepat adalah respons tepat dalam kegawatdaruratan dan penanganan wabah.

Kemarin saya terpicu pernyataan Dekan Fakultas Kedokteran USU Medan Profesor Dr.Aldy S.Rambe sebagai salah seorang narasumber pada Kongkow Virtual Seri-4 IKA USU Family pada 18 April 2020 dengan thema “Indonesia Menang Melawan Covid-19”.

Pak Dekan FK USU mengatakan “bahwa benteng terakhir kita yaitu petugas kesehatan di Rumah Sakit dan Unit Isolasi sudah mulai lelah”. Terbatasnya masa istirahat, isolasi dan pergantian jaga berhadapan dengan semakin banyaknya pasien baru terdampak Covid-19 setiap harinya.
Situasi ini mengancam daya tahan benteng terakhir.

Diakhir Maret, para Ketua Organisasi profesi kedokteran dan kesehatan termasuk IDI dan PPNI sempat memberikan Pernyataan Tertulis, “meminta anggota profesinya untuk sementara tidak ikut melakukan perawatan penanganan pasien covid-19 demi melindungi dan menjaga kesehatan sejawat”. Hal ini dilatarbelakangi kematian puluhan Dokter dan Perawat sejak Februari hingga kini.

Banyak kisah pilu dibalik semua peristiwa heroik para petugas kesehatan, yang bekerja tanpa menyadari bahwa ternyata pasien sudah terpapar virus Covid-19. Bahkan sebahagian ada Pasien berbohong.

Bapak Menkes Yth,
Pada surat kedua ini, perkenankan mengusulkan 2 langkah paska penetapan PSBB yang dapat mempercepat penanganan Covid-19 sekaligus juga mencegah kematian lebih banyak Petugas kesehatan.

LANGKAH KEDUA ini adalah : TOTAL CASE FINDING

Saatnya MENGUTAMAKAN pendekatan Pencegahan dengan melakukan PENEMUAN KASUS secara TOTAL dan FAKTUAL, kemudian dilakukan TRACING yang cermat.

Setidaknya ada 2 sumber yang perlu diwaspadai. Dari data kasus yang ada bisa ditelusuri yang Imported case (terbawa perjalanan dari luar negari) dan community spread (penyebaran dimasyarakat).

Dari 6.575 kasus terkini, bisa ditapis sumber infeksinya dan diikuti kluster dari kasus ini. Dan sekaligus mencegah fraud/kebohongan pasien.

Akar data ada pada riwayat kontak kasus.

Dalam hal ini Bapak Menkes berkenan meminta bantuan Menkumham/ Ditjen Imigrasi MENGELUARKAN SEMUA DATA transito (pasport) orang dan barang baik WNI maupun WNA ke Indonesia melalui 135 pintu masuk, SEJAK 1 Februari hingga orang terakhir masuk sebelum 135 pintu masuk resmi ditutup.

Pada Rapat Dengar Pendapat (RDP) Komisi II DPRRI 1April yl, Menkumham mengingformasikan Jumlah WNA Masuk Indonesia sejak Januari 2020 berjumlah 772.000 orang, dengan urutan terbanyak dari China 188.000, Australia 120.000, Singapura 130.000, sisanya Malaysia, India, Jepang, Korea Selatan, AS, Inggris, dan Rusia.

Bapak Presiden pada Ratas akhir Maret yl mengingatkan akan adanya perlintasan ratusan ribu sampai jutaan WNI yang kembali karena pemberlakuan lockdown/karantina di negara yang ditinggalkan.

Presiden minta melindungi kesehatan WNI yang kembali dan melindungi kesehatan masyarakat yang berada di tanah air.

Prinsip surveilans epidemiologi, semua yang diperhitungkan bisa mencapai 2.000.000 WNI dan WNA masuk Indonesia sejak Januari .

Untuk mencapai hasil penelusuran dan pemetaan serta konfirmasi medik, maka Kemenkes dapat bekerjasama dengan BIN, BIS, Kemendagri/Ditjen Adminduk, BKKBN/Deputi Adpin, BPJS, Badan SiberNas dan Telkomsel (untuk penelusuran No.Hp) agar duduk bersama untuk mendapat data bersih (saya sebut istilahnya) ORANG LAYAK DIKETAHUI (OLD).

Selanjutnya dilakukan Test Konfirmasi Virus.
Hasil konfirmasi laboratorium nantinya, akan diketahui status setiap orang beserta kontaknya, apakah ODP, PDP, Kasus Positif atau Sehat.

Inilah daftar tantangan nyata, dan MANAJEMEN KESEHATAN memiliki kemampuan untuk melakukannya.
Pemetaan kasus ini sangat amat penting.
Hanya dengan pemetaan yang tepat kita bisa bertindak tepat dan cepat, sekaligus memutus transmisi virus dan menghentikannya.

LANGKAH KETIGA adalah MEMBUAT OBAT & VACCINE

Untuk melindungi segenap Bangsa dan seluruh tumpah darah Indonesia, belajar dari himpitan virus Covid-19 ini, sudah waktunya Indonesia membangun kekuatan dalam bidang kedokteran dan pengobatan untuk mencapai kemandirian mengawal 270 juta penduduk Indonesia yang mendiami 17.504 pulau dari Merauke hingga Sabang.

Kemarin Menteri BUMN kaget bahwa kita masih mengimport 90% bahan baku obat-obatan.

Perhatian Menteri BUMN bisa dioptimalkan untuk mengembangkan indutri obat-obatan Konvensional dan sediaan Kesehatan Tradisional Khas Indonesia.

Membangun industri Vaccin sudah mendesak.
Wabah virus sangat merepotkan karena mengenai jumlah orang yang banyak dalam wilayah yang luas dan berdampak keberbagai aspek kehidupan mendasar, sehingga menimbulkan goncangan ekonomi, sosial dan tatakehidupan.

Dengan telah terjadinya kejadian wabah flu burung, Demam berdarah, SARS, Mers-cov dan kini Covid-19, maka kita sudah punya Virus nya dan seharusnya kita bisa membuat Vaksin.

Upaya yang sudah dirintis dimasa lalu dan berbagai terobosan mikrobiologi dan virologi  di berbagai Universitas dan Lembaga penelitian sudah bisa dikonsolidasi untuk membuat Indonesia UNGGUL Vaksin.

Insya Allah harapan bapak Presiden Jokowi untuk pengujian yang intensif dan pelacakan yang agresif, bisa kita laksanakan.

Ini adalah legacy mulia bagi negara yang melindungi segenap warga negaranya dan tumpah darahnya.
